Atletico coach Diego Simeone said on Tuesday that Mandzukic, who suffered a cut to the face in the first leg, is likely to be fit.
"We now have another training session where we expect him to be OK and I will base my plan, which I have fairly clearly in mind, around that," he said.
Real are also without midfielder Luka Modric, who suffered a knee ligament strain against Malaga and is expected to be unavailable for six weeks. Left-back Marcelo is suspended.
Boss Carlo Ancelotti said: "I have the best squad in the world and it is important to focus on the players who are going to play and not on those who are not playing.
"The answer will be to play fresh players, who are enthusiastic, fit, and who can help the squad.
"We have not beaten Atletico this season because they are a very strong side with one of the most solid defences in the world. It is difficult to score goals against them.
"It will be a close game, hard-fought in all areas and intense. I do not think that Atletico will be in a hurry to score and neither will we."
Atletico will be without suspended midfielder Mario Suarez against their neighbours in a repeat of last year's final, which they lost.

Striker Traore and midfielder Bakayoko both have leg injuries.
Arturo Vidal, who scored Juve's goal from the penalty spot in the first leg of this Champions League quarter-final, is a doubt with tonsillitis.
Paul Pogba (hamstring) and defender Martin Caceres (ankle) will also be missing for the Serie A leaders.
It has been over a decade since either of these teams reached a Champions League semi-final, with Monaco last in the final four in 2004, and Juventus in 2003.
Leonardo Jardim's Monaco caused a shock in the previous round by knocking out Arsenal and are third in Ligue 1, despite financial cutbacks at Stade Louis II.
"Right now, you could say that I am very proud of my players," Jardim told BeIN Sports.
"We've shown that we don't need to make big investments to go far in this competition.
"The club changed its strategy and the project in the summer. The players have stayed ambitious but the onus was placed more on the development of young players."
Juventus manager Massimiliano Allegri is expecting another tight encounter with the French outfit.
"The clean sheet at home gives us a slight advantage," he said. "Even so, it won't be straightforward.
"We now know our opposition - they're very well organised in defence and possess technique and physicality in forward areas.
"I'm optimistic, even though we know we'll have to put in a good performance and score to reach the next round."
